Job Overview
We are seeking dedicated and compassionate Asthma Community Health Workers to serve as a vital link between healthcare providers and the community.

The Community Health Worker – Asthma Educator plays a critical role in the delivery of asthma services under the Social Care Network (SCN). The CHW is responsible for conducting in-home asthma assessments, delivering evidence-based asthma self-management education, and facilitating access to home remediation services to reduce environmental triggers. This position requires strong community engagement skills, cultural humility, and the ability to empower clients and caregivers to improve asthma control and overall quality of life.

CHW – Asthma Educator will be expected to support their organization and will serve as a liaison to residents of across the five boroughs including Brooklyn, Bronx, Queens, Manhattan, and Staten Island. CHW – Asthma Educator will play a critical role in supporting management of Asthma and their home environmental conditions.

Responsibilities

1. Asthma Home Assessments

  • Conduct comprehensive in-home asthma assessments using SCN-approved tools and protocols.
  • Identify and document environmental triggers (e.g., mold, pests, carpeting, ventilation) that exacerbate asthma symptoms.
  • Assess household conditions, tenant concerns, and housing history to determine eligibility for home remediation services.
  • Photograph and document home conditions as part of the member case file.

2. Asthma Self-Management Education (ASME)

  • Provide structured, culturally competent asthma education in the home or via virtual platforms.
  • Educate clients and caregivers on asthma basics, medication adherence, proper inhaler/spacer use, and trigger reduction.
  • Utilize teach-back methods and visual aids to ensure comprehension and skill-building.
  • Assist members in developing individualized Asthma Action Plans in collaboration with their primary care provider and/or specialist.
  • Help people understand their health condition(s) and develop strategies to improve their health and well-being
  • Deliver health information using culturally appropriate terms and concepts

2. Care Coordination & Referrals

  • Facilitate referrals for SCN-funded home remediation services, Integrated Pest Management (IPM), and other supportive services.
  • Coordinate with landlords, NYCHA, and environmental vendors to implement remediation.
  • Serve as a liaison between the client, CABS’ care management team, and community partners.
  • Ensure continuity of care and follow-up with members post-intervention to assess progress and address ongoing needs.

3. Documentation & Reporting

  • Accurately document all encounters, assessments, education sessions, and referrals in CABS’ designated health record systems.
  • Submit member files, home assessment findings, and follow-up reports within required timelines.
  • Track outcomes, service delivery milestones, and satisfaction metrics in compliance with SCN standards.

4. Community Engagement & Health Promotion

  • Participate in community health events, workshops, and outreach efforts to raise awareness about asthma services.
  • Support recruitment and retention of SCN-eligible asthma clients, especially within public housing and high-need zip codes.

Qualifications

  • Experience in nursing, social work, public health, or related fields with a focus on community engagement.
  • Knowledge of medical terminology, asthma management techniques, and Medicare policies is highly desirable.
  • Strong communication skills, including public speaking and teaching abilities, to effectively engage diverse audiences.
  • Familiarity with data collection methods and basic analysis tools to evaluate program outcomes.
  • Experience working with individuals with disabilities or chronic health conditions is preferred.
  • Proficiency in motivational interviewing techniques to foster positive behavioral change.

Working Conditions

  • Fieldwork required in client homes across the five boroughs of New York City, Manhattan, Queens, Brooklyn, Bronx, Staten Island;
  • PPE and safety protocols provided.
  • Must be able to climb stairs and carry light educational materials or toolkits.
  • Occasional evenings or weekends for outreach events.

Minimum Qualifications

Required:

  • High School Diploma or equivalent; Associate’s Degree in Health or Human Services preferred.
  • Completion of Community Health Worker Core Competency Training or equivalent certification.
  • 1+ years of experience in health outreach, or home-based services.
  • Knowledge of asthma management principles, social determinants of health, and community resource
